We also work with Local Area Coordinators and lots of service and support providers and we collect information to assist with new plans and reviews. And we do all this so you don’t have to worry or lift a finger. We can help you if you have Support Coordination, Improved Daily Living, Increased Social and Community Participation, and/or Improved Living Arrangements identified in your NDIS plan and it doesn’t matter whether you have chosen to be agency-managed, plan-managed or decide to self-manage your NDIS plan funds…. Did you know we don’t just work in the disability sector we live in it. SchemeWise is a business built to specialise in NDIS Support Coordination and we are parents of children whom use the NDIS too. We know what works and what doesn’t and we also know what its like to live with a family member who lives with disability and the concerns around ensuring their ability is maximised.
SchemeWise also offers a number of value-add services which you will see on our website. In Canberra we offer nurse-based assessments, and supports, which can help you get the information needed to assist in NDIS planning discussions, provide more information to support eligibility or help you work out the best support options to achieve your goals. Our nurses can also help you work through the planning process before you sit down with a Local Area Coordinator and help you understand what is needed at a planning meeting and the supports and/or services which might help you under an NDIS plan.
We can also assist you by providing a qualified counsellor to help you with goal-setting and identifying actionable items or even if you just need personal assistance in your life. Best of all OUR COUNSELLORS travel to you and can meet at your home (if it is safe to do so), or locally in your community (like a coffee shop or local park)! Our counsellors can also help carers and family members to ensure that supporting someone with disability doesn’t become overwhelming with sessions available singly or in small group settings…
