SchemeWise Nursing Services

Did you know SchemeWise provides two major types of nursing services?

Community Nursing – under the NDIS system community nursing is a specialised service for people living with disability.  State and territory governments’ no longer offer public commuity nursing services and it hterefore needs to be funded under your NDIS plan.  the good news is that SchemeWise nurses are expereinced community practitioners and can therefore support you whether you live in ACT or Southern NSW NDIS areas.  From pressure sores, to medication injections, wound management, continence and neurological issues our community nurses can support you!

Assessment and Support by a Nurse – SchemeWise registered nurses can also provide short term help if your circumstances change and there is a need for more information to be exchanged with your services and supports, if you need an assessment report to support your plan or plan review, where additional training for your carers is required, or even where your circumstances are changing but not stabilised.  For example, if things have changed but your GP, specialist or other therapists have not described how a health or disability-related issue affects you on a daily basis a nursing assessment is worthwhile getting.  If your NDIS plan includes support from allied health professionals and you live in the ACT or Southern NSW NDIS areas then our nurses can help!

SchemeWise also accepts referrals for nursing assessment and support or community nursing assistance from LAC’s, Doctors, Hospitals and other community services.  It takes a community to care after all!!
